Set Up Your Appointment

You can schedule your appointment online or by calling our office at 980-249-3993. When scheduling your visit, please also provide some basic information to ensure we can serve you efficiently.

What is basic information?

Basic information to provide our dentist and team includes:

  • Personal information, such as your full name, date of birth and phone number.
  • Insurance information, including the name of your insurance provider, the policy number or member ID, group number (if applicable) and name of the primary insured if it is not you.
  • The reason for the appointment (general checkup, a follow-up visit, a specific concern, etc.)
  • Medical and dental history – past procedures and treatments, known allergies, any current medical conditions or medications, etc.
  • Emergency or special needs – is your appointment urgent? Do you require sedation, mobility assistance or other accommodations?
  • Preferred appointment details, such as your desired date and time and any alternative dates or times for your visit.

Please arrive 10 minutes before your scheduled appointment time. This will give you time to fill out any forms or other paperwork that may be needed and will help ensure your appointment runs smoothly and on-schedule.

What will I have to pay at my first visit?

Payment at the time of your initial visit will depend on several factors, such as the services provided during your visit and your insurance coverage. Many dental insurances require a co-pay for routine visits or specific treatments. This co-pay may be a fixed fee or a percentage of the cost of the visit. If your insurance does not cover the full cost of your initial visit, you may need to pay the difference out-of-pocket. We will work with you to help you understand your insurance benefits and coverages, and encourage you to speak with your insurance provider as well.

If you do not have dental insurance, you may need to pay the full cost of your visit at the time of service. Our practice offers a membership plan for patients without insurance, and we will be happy to discuss the details of this plan with you.

Our practice also accepts most PPO dental insurances and offers other financing options through CareCredit®. Please view our Financial Options page for more details.

Your Exam & Consultation With Dr. Pollack

Dr. Pollack will perform a through exam and present a treatment plan tailored to your needs. This exam will include a discussion of your current oral health needs and smile goals.

What do our treatment plans look like?

Our treatment plans outline the recommended dental procedures based on your oral health needs, including preventive, restorative and cosmetic treatments. Your plan will detail the steps involved, estimated costs, expected outcomes and the timeline for completion of your treatment. These plans often prioritize treatments by urgency, starting with immediate concerns like decay or gum disease before addressing elective procedures. Treatment plans also include insurance coverage details, patient responsibilities and follow-up care instructions. By providing a clear roadmap, your plan can help you make informed decisions and understand your role in achieving optimal oral health.
